Covid-19 PCR testi önerileri

KBB hekimi olan Prof.Dr. Orhan Özturan'dan Covid-19 PCR testi için kültür alınmasının etkinliği konusunda bir öneri:
Covid-19 için yapılan testler sıklıkla kişi hasta olsa da negatif sonuçlanabilmektedir. Bunun önemli nedenlerinden biri kültür materyalin alınmasındaki teknik zorluklar ve başarısızlıklardır. Virusun en yoğun bulunabileceği nazofarenks denilen geniz bölgemizdir. Burun yoluyla nazofarenksden yani genizden kültür alınması kolay bir işlem değildir. Bu işlem sırasında hasta bir miktar acı çeker ve rahatsız olur. Ayrıca bu kültür işlemi, kültür çubuğunun sırasıyla her iki burun deliğinden genize kadar sokulması ile alınmaktadır. Her iki burun deliğini kullanarak ulaşılmak istenen yer aslında aynı bölgedir.
Birçok insanda burun içindeki kıkırdak ve kemik eğriliği nedeniyle burnunun bir tarafı, diğerinden daha dardır. Üstelik, nazal siklus denilen fizyolojik refleks nedeniyle burun boşluğundaki alt konka denilen etler dinamik bir yapıya sahiptir. Her iki taraftaki etler münavebeli olarak saatler içerisinde yavaş yavaş büyür ve küçülürler. Normalde burun tıkanıklığı hissetmemize rağmen her iki burnumuzdan geçen hava ve burnumuzun iç genişliği bahsi geçen nazal siklus sayesinde zaman içerisinde farklılıklar gösterir.
Bu iki durumu dikkate alarak kültür alınacak hastaya, başparmağının içteki yumuşak tarafı ile burun deliklerini sırasıyla hafifçe kapatarak diğer burnundan nasıl nefes aldığı daha önemlisi hangi burun deliğinden daha rahat nefes alabildiği sorulmalıdır. Genizden burun yoluyla alınacak kültür çubuğu, hastanın ifadesi ile daha rahat nefes aldığı taraftaki burun deliği yoluyla içeri sokulmalıdır. Kolaylıkla nazofarenkse ulaşıp burada kültür çubuğu biraz çevrilerek yeterli materyal güvenle ve olabildiğince acıtmadan alınabilir. Ayrıca dar olan diğer taraftan zaten aynı bölgeye ulaşma çabasıyla hastaya acı verecek ve zaten başarısızlığa aday bir kültür alma işlemi anlamsızdır.
Ağız yoluyla boğazın arka duvarından da kültür alınabilir, ancak geniz bölgesi virusların burada daha yoğun yerleşmiş olması nedeniyle teşhis için daha önemlidir.
Nazofarengeal kültür materyalinin alınmasında diğer bir önemli konu, hastanın kafasını ekstansiyona getirmeden burun ucunu sol elin başparmağı ile hafifçe yukarı doğru kaldırırken kültür çubuğunun burun tabanına paralel olarak 9-10 cm kadar ilerletilmesi ve ve nazofarenks arka duvarına temas ettirilmesidir.
Bu öneriler kabul görülerek uygulandığı taktirde PCR test sonuçlarının yeterli materyal temini sağlayarak güvenli, etkin ve hastayı en az rahatsız ederek daha doğru çıkacağı ve gerçek hastaları tespit edilebileceği kanaatindeyim.

A suggestion from an ENT doctor, Prof.Dr.  Orhan Ozturan on the effectiveness of taking culture for Covid-19 PCR test:
PCR tests for Covid-19 may often result in negative, even if the person is ill.  One of the important reasons for this is the technical difficulties and failures in taking the cultural material through the nose.  Postnasal region called nasopharynx where the virus can be found most intensely.  It is not an easy procedure to take a culture from the nasopharynx through the nose.  During this procedure, the patient suffers some pain therefore may be uncooperative and it is quite uncomfortable and nasty intervention. For the test some secretion is taken by inserting the culture stick through both nostrils to the nasal passage reaching the nasopharynx, consecutively.  The place to be reached by using both nostrils is actually the same area.
In many people, one side of the nose is narrower than the other because of the cartilage and bone curvature in the nose.  Moreover, due to the physiological reflex called nasal cycle, the soft tissue called lower turbinate in the nasal cavity have a dynamic structure.  These soft tissues on both sides gradually grow and shrink  gradually within hours by the control of autonomic nervous system. While the one turbinate in one nose enlarges, the other one shrinks physiologically. Although we normally do not feel any nasal obstruction, the air passing through both our noses and the inner width of our nose vary over time due to the nasal cycle process mentioned above.
Considering these two conditions (septal deviation and temporal turbinate changes due to nasal cycle) , the patient should be asked about which nostril he/she can breathe more easily by closing one nostril by the inner soft tissue side of the thumb slightly while breathing through the other open nostril consecutively.  After discovering the more open favorable nostril, the culture stick is taken. It should be inserted through the nostril on the side where the patient breathes more easily with the statement of the patient in response to consecutive closure with thumb and getting the answer which side is more open.  By reaching the nasopharynx easily through the more open side and turning the culture stick a little bit there, enough material can be taken safely and without hurting as much as possible. Whereas any attempt of taking nasopharyngeal culture from the narrow side of the nose would be a  painful and futile intervention and possibly unsuccessful. Besides, a trial to reach the same area from the narrow side may be a meaningless trial that may be a candidate for failure.
Cultures can also be taken from the back wall of the throat by mouth, but the nasal region is more important for diagnosis because viruses are more concentrated in the nasopharynx.
Another important issue in the removal of nasopharyngeal cultural material is; nasal swab is advanced up to 9-10 cm or even more parallel to the nasal fossa while lifting the tip of the nose slightly upwards with the thumb of the left hand without extension of the patient's head. The tip of the swap must touch to the posterior wall of nasopharynx.
I believe that if this recommendation is accepted and applied, PCR test results will be more accurate, safe, effective and less disturbing to the patients by providing sufficient and right test material supply and eventually true patients can be identified.
